10 Early Warning Signs of Syphilis You Should Never Ignore

Nexus Laboratories health information team • Updated 8 October 2026

Syphilis can be missed because symptoms may be mild, hidden or absent. These ten possible warning signs are reasons to seek assessment, not a checklist that diagnoses infection. Some occur later or indicate complications rather than early disease.

Ten possible warning signs

A genital sore; a sore around the anus; a mouth sore; a rash on the palms; a rash on the soles; swollen lymph nodes; unexplained patchy hair loss; fever with a rash; new vision changes; and new hearing changes can warrant assessment. Several are nonspecific, and many people with syphilis have none of them.

Vision and hearing changes cannot be assessed from these photos. Seek urgent medical care for new changes in vision or hearing.

Why appearance is not enough

A syphilis sore is often painless, but sores can look different and may be inside the mouth, vagina or rectum. A sore healing or a rash disappearing does not establish that an infection has cleared. Other conditions can produce similar symptoms.

What to do next

Arrange a clinic visit for a new sore or unexplained rash and share your exposure history. Seek urgent care for vision or hearing changes. Do not wait for every sign to appear, use photographs to diagnose yourself, or take leftover antibiotics.

Frequently asked question

Can I have syphilis with none of these signs?

Yes. Symptoms alone cannot rule it out. A clinician can advise testing based on your exposure history.

Useful next steps for readers in Malaysia

Use this guide to prepare questions for a healthcare professional. Bring your exposure dates, symptoms, test results and any treatment records. Ask what the result means, whether you need another test and how follow-up will be arranged.

This is general health information, not an individual diagnosis. Seek urgent care for new vision, hearing or neurological problems; contact your antenatal team promptly about a possible exposure during pregnancy.
